Download the official PDF form for your request. Fill in all required member details, account numbers, and sign either with a digital signature or handwritten.
Attach Supporting Docs
Attach 2 valid forms of government ID (National ID, Passport, or Driver's Permit), a recent utility bill dated within 3 months, and your latest payslip if applying for loans.
Submit for Processing
Email your completed forms and attachments to info@cbecu.com, or submit in-person at our credit union office on the 11th Floor, Central Bank Building.
